restraining order,…If a person has a restraining order placed on them

If a person has a restraining order placed on them, however they were never found to get served papers, what is the outcome of the order?

If one does get served papers, does it cost any money to have to go to court?

If one does get served papers, how does it effect their record?

You must have been served in some way or another (unless the individual really court not find you and was able to serve you a different way) in order for there to be a restraining order against you. That being said, the court will NOT issue one absent the showing that there was service on you. You do not have to actually be in court for the restraining order to be issued.
It does not cost any money to go to court to defend yourself in a restraining order action unless you hire a private lawyer.
Having a restraining order can effect your record as it is public information so potential employers may find out about it.
