Is a bar responsible for all drinking int it’s establishment? What if someone who is underage, but looks very old and has a legitimate identification? What if someone sneaks alcohol in and spikes his or her non-alcoholic drinks? Is the bar liable?

Yes, pretty much the bar is screwed in this type of situation.
Yes the bar is ultimately responsible and liable for the patrons whom are drinking it it’s establishment. The restaurant/bar is also responsible that everyone is of legal age, regardless of how old they look or how realistic their age identification looks. In addition, at least in the state of New Jersey, a bar can be help liable for underage drinkers sneaking alcohol into a the bar, getting drunk, and then crashing their car. The New Jersey Supreme Court just help a bar liable in just a scenario.
